Goal
To modernize Kaiser Permanente's Home Care program by connecting disconnected systems and introducing patient-facing tools that improve provider efficiency and patient self-management.
Objective
Design a proof-of-concept product suite that centralizes care, documentation, and scheduling for providers, patients, and caregivers."

Collaborated with the product team to gather and synthesize stakeholder feedback, using findings to inform the design direction.
The prototype was iterated in near real time after each group session, using direct feedback to explore alternative solutions. Testing insights drove design pivots that improved usability, better supported providers in real-world scenarios, and defined the MVP.


The dashboard consolidates real-time Command Center updates, patient information, and communication tools, giving providers everything they need to manage appointments from one place.


Though the iPad included a keyboard attachment, the app was intentionally designed to minimize typing. Quick, tap-based actions allow providers to complete data entry faster and dedicate more time to patient care.

Providers can select clinical outcomes and access intervention and summary history, improving documentation accuracy and supporting better-informed care decisions.
Designed the Dynamics 365 Command Center workflows and UI, integrating real-time appointment updates and communication tools to streamline care coordination for support teams.


Selected members received Android tablets with features designed to build accountability and strengthen engagement with their care team.
The proof-of-concept revealed measurable data on patient engagement and care delivery, giving the organization the evidence needed to define its home health transformation strategy.
