Goal

Transform a fragmented Home Care ecosystem into a connected digital experience by integrating disconnected systems and workflows, simplifying complex interactions across multiple users, and creating tools that improve operational efficiency and self-service.

Objective

Design a connected product ecosystem that centralizes care, documentation, and scheduling across providers, patients, and caregivers. Create a cohesive experience across multiple users, workflows, and platforms while working within the constraints of existing enterprise systems and technology.

Care At Home Prototype

Discovery

Conducted discovery sessions with users and stakeholders to understand current workflows, pain points, and system constraints. Synthesized findings into key insights and opportunities that shaped the product direction.

Test, Learn, Pivot

Conducted two days of hands-on user testing with the product team and stakeholders, iterating on interactive prototypes based on feedback. Testing revealed that iPad placement and keyboard use disrupted patient care, leading the team to explore a hardware solution better suited to the physical workflow.

Streamlined Task Management

One of the biggest pain points for home care providers was managing appointments and coordinating with patients on their own. Without a centralized system, providers relied on phone calls and text messages to schedule visits, communicate changes, and coordinate care.

I designed a centralized experience that brought appointment management, patient information, communication, and real-time Command Center support into one workflow, giving providers a single place to manage their schedules and coordinate care.

Minimized data entry

The prototype relied on the existing EHR input flow, which introduced unnecessary steps for providers. I consolidated the data-entry workflow and intentionally minimized typing to make documentation faster and easier.

Quick, tap-based actions allowed providers to complete data entry more efficiently, reducing interaction overhead and allowing them to dedicate more time to patient care.

Streamlined Documentation

Providers often had to complete Epic documentation after visits or after work hours, adding to their administrative burden. The existing workflow required extensive manual data entry and multiple steps.

I streamlined documentation with quick, tap-based interactions for selecting clinical outcomes and accessing intervention and summary history, reducing data-entry effort and allowing providers to spend more time with patients.

Dynamics 365 Command Center

Designed a new Command Center in Dynamics 365, creating workflows and UI that provided support teams with real-time appointment visibility and centralized communication for more efficient care coordination.

Alerts and Notifications

Patient Engagement

Designed an Android tablet experience that gave patients a centralized way to manage their care and stay connected with their care team. Features included messaging, task management, appointment scheduling and follow-up, and gamification to encourage self-care and medication adherence.

Outcome

The proof of concept generated measurable insights into patient engagement and care delivery, helping define the product direction and broader Home Care transformation strategy.
